Clinical Case Form

To be eligible for a GDT $200 Gift Card, your submission must include all necessary images (X-rays and standard photos of the patient before, during, and after the treatment) and a detailed description of the procedure (case notes). Additionally, the procedure must incorporate products by "GDT Dental Implants" or "GDT Dental Supplies" brands.
